12/28/2023 EnchantaVaughn Approved (AM) Upon arrival, client was seen focusing on her asssignment upon arrival to the Winter Program. Client communicated that she was feeling good about herself and woke up in a great mood. Client and appeared to be having a wonderful morning, as evidenced by her positive tone and energetic demeanor. (PM) Client was observed in class on task and displaying excellent pro social behaviors (e.g. helping her peers during group activity, sharing her school supplies when needed, and volunteering to pass out classwork to the class). Client talks a lot about her unwanted feelings and expressed that she had a bad dream but didn’t let it effect her day. Client was seen displaying appropriate manners in the lunch room, politely standing in line and grabbing her food when it was her turn. Client interacted well with her peers and was able to transition back into class in an optimistic mood. (EOD) Client was cooperative and attentive to her teachers prompts and requests. Client remained on task demonstrating excellent self control and personal responsibility. Client met objectives 1, 2, and 3, as evidenced. (AM) QMHP welcomed client to Winter Program and provided support and encouragement to ensure a good start to her day and to be on track with achieving her goals and objectives. QMHP monitored client individualized behaviors in collaboration with her teacher and other mental health professional. (PM) QMHP reminded client to utilize self calming techniques (i.e. counting and deep breathing) during testing. QMHP brainstormed ways client can make herself better. QMHP isolated client from aggressive behaviors to decrease disruption in class and to reduce excessive talking. QMHP processed unacceptable behaviors and discussed possible consequences. (EOD) QMHP taught client alternative method to interpret and cope with stress in the classroom by reevaluating situation or problem in a more positive way. QMHP educated client on coping strategies to deal with her feelings and emotions. QMHP supported client with creating a hobby to engage in during free time and individual breaks (i.e. writing a story or coloring a picture). QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve her goals and objectives. QMHP reviewed psycho educational activities to ensure retention and knowledge gain. QMHP provided praise when the client continued to display positive behaviors. QMHP explored client feelings about different subjects in school and how to manage her confidence level and self esteem. (AM) Client greeted QMHP with a hug and said hello to peers. Client verbalized positive feelings about finishing her work and expressed that she feels inspired to be the voice of her school. Client nodded to staying on top of her goals and objectives. Client displayed high level of participation in class, lunch, and gym settings. (PM) Client responded in a respectful manner with acknowledging the reinforcement that the QMHP provided by nodding in agreement. Client responded well to feedback. Client was acceptive and motivated to make herself better each day. (EOD) Client expressed in order to stay calm she takes a moment to stretch and will begin thinking about her personal strengths. Client displayed gratitude toward her counselors and returned back to class in an optimistic mood. Client responded well to review and demonstrated understanding. Client said "thank you".
